The change is real. A Phase 2 trial published in the New England Journal of Medicine in 2023 by Jastreboff and colleagues reported roughly 24 percent mean weight reduction at 48 weeks in the highest arm, in adults with obesity. Losing close to a quarter of body weight is visible from across a room. It is also a drug approved in no country, with no prescription route and nobody who can lawfully supply it, which is the most decision relevant fact on this page and the reason it appears in the second paragraph rather than the last.

What no image from that process can show is which tissue left.

What the trial can tell you that a photograph cannot

A randomised trial has a control group. That is the entire difference, and it is not a technicality.

Without one, every apparent change competes with the other things that were happening at the same time: a new training block, better sleep, the seasonal habits of someone who decided in January to sort themselves out, and the ordinary variation of a body across a year. With one, the comparison is against people doing everything else the same and receiving placebo.

That is why the published figure carries weight and an album does not, even when the album is more persuasive to look at.

What a photograph can tell you that the trial cannot

Photographs are not worthless. They record shape, which a mean weight reduction does not, and shape is what most people actually care about.

They are also honest about one thing the numbers hide: how a specific individual responded. Trial means conceal a wide spread, and every image is at least a real data point about one body, even if it is uncontrolled and unverifiable.

Body composition substudies of related incretin drugs in humans have reported that a meaningful share of total weight lost is fat free mass rather than fat. That category is mixed. It contains water, glycogen and the fluid bound to it, gut contents and organ tissue as well as contractile muscle, so treating the whole figure as muscle overstates the loss considerably.

It does not reduce it to nothing. Over a large, sustained deficit, part of what leaves is muscle, and muscle is rebuilt only by training and feeding rather than by returning to normal eating.

In a photograph these are indistinguishable. A leaner arm with less muscle under the fat and a leaner arm with the same muscle under less fat look similar in a phone picture taken in different lighting eleven months apart. That is not a criticism of the person posting. It is a limit of the medium.

The timeline, as far as it is known

The published result is anchored at 48 weeks. That is the number to hold on to, because it sets the scale of the commitment: this is a year, not a cutting block.

Weight in this drug class comes off progressively rather than in a step, and the earliest visible changes in any large deficit are partly water and glycogen. Beyond those general statements, no timeline for this compound should be quoted with confidence, because doing so would mean inventing detail the published work does not contain.

Why lifters notice the face and arms first

Fat distribution is not uniform, and neither is loss from it. In a large deficit the face, neck and forearms tend to show change early because they carry less subcutaneous fat to begin with.

The result is the drawn look that gets discussed online. Part of that appearance is fat loss from places where fat was doing structural work. Part of it, in someone who was carrying real muscle, is the loss of tissue underneath.

For a lifter this is the moment worth paying attention to, because it is the one point where the mirror gives an early signal. It is still not a measurement.

Two people, the same end weight

Picture two men of the same height who each finish a year at the same body weight, both from the same starting point.

The first kept training three or four times a week throughout, kept protein high enough to be inconvenient, and lost most of the weight from fat. He is lighter, visibly leaner, and his working sets are close to where they were.

The second stopped lifting somewhere around month three because he felt flat and food had become uninteresting. He lost the same total mass, a larger fraction of it lean, and he now weighs what the first man weighs while carrying noticeably less muscle under a similar amount of fat.

On a scale they are identical. In a before and after photograph taken in flattering light they may look broadly similar, because both are much smaller than they were. In a body composition scan, a strength log or a photograph taken two years later, they are not remotely the same outcome.

The drug did not decide which of those two happened. The training and the eating did, and neither of those appears in the picture.

What an honest before and after would require

The same person, the same lighting, the same time of day, the same hydration state and the same posture. Body composition measured on the same machine at both ends rather than a scale weight. A record of training volume and protein intake across the period, since both change body composition independently of any drug. And a strength log, because a person who lost a quarter of their body weight and lost their working sets with it has not achieved what the picture suggests.

Almost nothing posted online meets even half of that. What is posted is selected, and the selection runs in one direction.

After the after

The photographs stop at the good end. That is the structural problem with the genre, and it is worse here than usual.

Discontinuation in this drug class has been followed by weight regain in humans. Fat regained on a surplus returns readily. Muscle lost in a deficit returns only through training and feeding, slowly, and more slowly still in an older lifter whose response to both stimuli is already blunted.

Whether regained weight comes back in the same proportions it left is an open question in this literature rather than a settled finding. But the asymmetry in how the two tissues rebuild is not in doubt, and it means the eighteen month picture could easily be worse than the starting one at an identical body weight. Nobody posts that photograph, because by then the account has moved on.
